PURPOSE: An undetermined number of patients with temporomandibular joint
(TMJ) symptoms have been treated with intra-articular disc implants
composed
of Teflon ethylene/propylene or Teflon polytetrafluoroethylene and
aluminum
oxide (Proplast-Teflon; Vitek, Houston, TX). These implants have shown
the
potential to fragment in situ resulting in nonbiodegradable particles
that
stimulate a giant cell reaction and lead to degeneration of local
structures, pain, and limitation of mandibular opening. We examined the
possible relationship between TMJ implants and persistent pain,
responses to
sensory stimuli, quality of life, and systemic immune dysfunction.
PATIENTS
AND METHODS: This case series (32 patients) were referred from
university-based orofacial pain centers and private practices from
across
the United States. Laboratory and clinical assessments evaluated
orofacial
pain symptoms, neurologic function, clinical signs and symptoms of
rheumatologic disease, physical function, systemic measures of immune
function, and behavioral measures. RESULTS: We found that TMJ implant
patients appeared to have altered sensitivity to sensory stimuli, a
higher
number of tender points with a diagnosis of fibromyalgia, increased
self-report of chemical sensitivity, higher psychologic distress and
significantly lower functional ability. Systemic illness or autoimmune
disease was not evident in this series of TMJ implant patients.
CONCLUSIONS:
Significant problems were noted on clinical assessment of TMJ implant
patients. This is a US government work. There are no restrictions on its
use.

Prezado Luiz Fernando,

Não sei se seria novo mas para pacientes com encefalopatia
eu costumo recomendar enemas de café, bastante lactobacilus
acidophilus e outros probioticos. A intenção e fazer os microrganismos
consumirem tanto a amonia quanto outras toxinas tornando-as
inofensivas.
Quero crer que o paciente em questão tem cirrose.
Nesse caso o SAMe é bastante indicado.

Obrigado pelas dicas ( a paciente tem uma cirrose expressiva e as
transaminases estão já muito baixas, o que sugere uma falência hepática em
andamento)

Estou estudando alternativas que encontrei nos sites tais como Benzoato de
Sódio (liga-se à Glicina e dá o Hipurato que se liga à amônia)  Encontrei
menções e sites (ver no Google zeolites) que falam do uso de bentonita e
outros minerais. Vou passar este sábado vendo os sites que você recomentou
de MCM

De nada.
Sim as argilas como a bentonita sempre tem
o poder de adsorver ( com "d" mesmo) as toxinas.
Nesse caso o carvão ativado talvez dê algum resultado
e também é barato. Tanto as argilas ( bentonita, zeolita e outras )
como o carvão ativado devem ser tomados entre as refeições
para evitar a adsorção de minerais como Ca, Mg, Zn, Se, etc...de
cargas positivas (cátions).
  A amonia (NH4+) tem uma carga positiva e portanto pode ficar
adsorvida em qualquer argila pois as mesmas tem uma infinidade de
cargas negativas . O que difere uma rgila da outra nesse aspecto é
a chamada C.T.C. ou Capacidade de Troca de Cátions.

Prezado Luiz Fernando,

Não sei se seria novo mas para pacientes com encefalopatia
eu costumo recomendar enemas de café, bastante lactobacilus
acidophilus e outros probioticos. A intenção e fazer os microrganismos
consumirem tanto a amonia quanto outras toxinas tornando-as
inofensivas.
Quero crer que o paciente em questão tem cirrose.
Nesse caso o SAMe é bastante indicado.

É tambem um alumino silicato como várias outras
argilas. Talvez seja mais eficiente para reter a amonia
que tem carga positiva em função da sua estrutura cristalina
ser diferente das outras argilas que geralmente tem uma estrutura
tipo baralho de cartas ou "mil folhas". A estrutura da zeolita
é fixa tipo casa de abelha e não sofre muita alteração com o
grau de hidratação. Tem elevada C.T.C.
Talvez voce encontre aqui no Brasil um produto mais
industrial. Para torna-lo ingerível deve ser primeiro esterilizado
com calor ( Colocar no forno e pronto - É assim que eu faço com as
argilas).
Tem sido muito usada para reter a amonia ou seja é uma argila
conhecida por essa caracteristica mas como diz o velho ditado
"quem não tem cão caça com gato". Caso não encontre uma zeolita
descente use qualquer outra argila.
Detalhe : Quanto menor a partícula de argila mais reativa.
Como eu faço para separar as menores ?.
Coloco tipo uma colher de sobremesa em um copo com agua
agito e deixo descansar por alguns minutos. Todas as partículas
maiores ( mais pesadas ) vão imediatamente para o fundo e eu
bebo somente o sobrenadante ou seja aquela agua com aspecto de agua
suja.
Esse tipo de partícula é altamente eficiente. Não creio ser interessante
consumir as partículas que não são capases de se manter em suspensão
na agua.

Creio que ela teve outros problemas de ordem pessoal

De nada.
Sim as argilas como a bentonita sempre tem
o poder de adsorver ( com "d" mesmo) as toxinas.
Nesse caso o carvão ativado talvez dê algum resultado
e também é barato. Tanto as argilas ( bentonita, zeolita e outras )
como o carvão ativado devem ser tomados entre as refeições
para evitar a adsorção de minerais como Ca, Mg, Zn, Se, etc...de
cargas positivas (cátions).
  A amonia (NH4+) tem uma carga positiva e portanto pode ficar
adsorvida em qualquer argila pois as mesmas tem uma infinidade de
cargas negativas . O que difere uma rgila da outra nesse aspecto é
a chamada C.T.C. ou Capacidade de Troca de Cátions.

Obrigado pelas dicas ( a paciente tem uma cirrose expressiva e as
transaminases estão já muito baixas, o que sugere uma falência hepática
em
andamento)

Estou estudando alternativas que encontrei nos sites tais como Benzoato
de
Sódio (liga-se à Glicina e dá o Hipurato que se liga à amônia)
Encontrei
menções e sites (ver no Google zeolites) que falam do uso de bentonita e
outros minerais. Vou passar este sábado vendo os sites que você
recomentou
de MCM

No site do Zhang ele ressalta, com muita propriedade, alguns aspectos
descuidados pela medicina ocidental.  Destes, o que me parece mais
importante é a abordagem reducionista, que dá tanta importância ao vírus que
se não leva em conta o papel do organismo na enfermidade.  Nas palavras dele

it has been demonstrated that prostaglandin E2 abrogates the induction of
PCA by MHV-3 both in vivo and in vitro and prevents hepatic necrosis and the
associated microcirculatory changes. These data suggest that MHV-3 induces
cellular injury through the activation of the coagulation cascade and

Esta perspectiva me levou a dar uma revisada nos artigos acerca dos efeitos
da hepatite na microcirculação hepática e realmente o chinês está
absolutamente certo. Há inúmeros trabalhos mostrando que a hepatite (viral
ou alcoólica) afeta significativamente a eficácia da microcirculação.  O
passo seguinte foi procurar recursos terapêuticos para lidar com isto e
deparei-me com alguns recursos, que eu já utilizava mas que não tinha

Protective effects of trifluoperazine on the microcirculation of cold-stored
livers.

Previous studies have shown a protective effect of trifluoperazine (TFP), a
calmodulin inhibitor, upon the microcirculation of cold-stored kidneys. The
present study points to similar beneficial effects of TFP on the
microcirculation of cold-stored livers; 25 canine livers were preserved for
24 hr with Euro-Collins' solution (EC) (n = 8), University of Wisconsin
solution (UW) (n = 7), or UW + TFP (n = 10). The stored livers underwent
heterotopic transplantation (HLTX); hepatic-artery and portal-vein pressure
and flow were monitored; oxygen consumption and extraction were measured
before HLTX and at 15-min intervals after reperfusion, for 1 hr. Mean
hepatic-artery and portal-vein flow (HAF & PVF) prior to donor hepatectomy
were 172 and 530 cc/min, respectively. Poor HAF and PVF occurred in EC-HLTX
(mean 35, 175 cc/min, respectively). The damaged EC-flushed livers could not
compensate to the decreased hepatic blood flow by increased oxygen
extraction (oxygen consumption and extraction, 8.7 vol.% and 48%,
respectively). Light and electron microscopy showed severe liver necrosis
and periportal hemorrhages. Improved hepatic-artery and portal-vein flows
were seen in UW HLTX (105 and 254 cc/min), and oxygen consumption and
extraction were 16.4 vol.% and 66%, respectively. Liver biopsy taken just
before reperfusion revealed well-preserved liver architecture. Liver biopsy
obtained 1 hr after reperfusion revealed marked edema of the portal triad,
sinusoid congestion, and hemorrhage. Electron-microscopy biopsies obtained
during reperfusion at 15-min intervals revealed severe vasospasm of the
terminal hepatic arterioles and progressive damage to the liver
microcirculation. The addition of TFP to the UW-flush solution resulted in
excellent protection of the liver microcirculation. Marked increase in
hepatic-artery and portal-vein blood flow was noted after reperfusion (mean
167 and 421 cc/min, respectively (P 0.02 vs. UW: P 0.001 vs. EC). The
recovery of metabolic activity was evident by the high oxygen consumption
and extraction (25.8 vol.% and 80%, respectively). And serial liver biopsies
obtained after reperfusion have shown excellent protection of liver
architecture and the absence of hepatic arteriolar vasospasm. Taken
together, these data suggest that the addition of TFP to the UW solution
protects the liver microcirculation by rendering the hepatic
microcirculation insensitive to vasospastic stimuli during reperfusion, thus
permitting better metabolic recovery after transplantation.

Obrigado.
Eu conheço o Dr Zhang pessoalmente. Estive
com ele na sua residencia in New Jersey em 1999.
Ele me causou uma grande impressão e me pareceu
um profundo conhecedor da TCM agora revivida como
MCM com o auxílio da ciencia e da tecnologia.
Eu tive apenas uma divergencia com ele  com relação a
erva que usava para "sedar" o sistema imunológico, na
época, o Trypterygium willfordii. Na verdade essa erva na
China é tão potente nesse aspecto que é usada no lugar
da ciclosporina para evitar rejeição de orgãos transplantados.
Eu tive uma polemica com ele na época e eu levantei a questão
de que não era necessário dar uma cacetada tão grande no
sistema imunologico para sedar. Ele não gostou muito da crítica,
na época, mas o fato de ter tirado o produto que continha essa
erva, do repertório dele ( o AI# 1), já me faz crer que eu fui ouvido,
de certa forma. Aliás essa é a minha função e porísso me chamam
de Ginger Joe. A minha função é fazer críticas "condimentadas"
para não dizer apimentadas e fazer as pessoas se mexerem.
Hoje ele utiliza o AI#3 que contem outras 3
ervas, as quais eu pesquisei e não encontrei nenhuma contra
indicação.
O fato dele ter tratado mais de 800 pessoas com HCV também ajuda muito.
Telefonei para ele há uns tres meses atras e ele ainda se lembrava
do "Ginger Man" do Brasil.
O livro dele também é excelente. Repete bastante o que existe no site
mas tem algumas pérolas como :
" Não existe nenhuma doença mais estudada na história da Medicina
e na qual se investiu maior soma de recursos doque a AIDS. No entanto
poucos resultados foram colhidos, o que se certa forma demonstra que
os cientistas estão no caminho errado".
Tentar matar o virus é o caminho errado. Nesse processo acabase matando
eventualmente o hospedeiro do virus.
O mesmo pensamento se aplica a Hep C e HCV.

excepcionalmente bom.  Abre perspectivas muitointeres, ainda mais que a
fundamentação (para mim que trabalho com bioquímica e biologia
molecular) é

Sim as argilas como a bentonita sempre tem
o poder de adsorver ( com "d" mesmo) as toxinas.
Nesse caso o carvão ativado talvez dê algum resultado
e também é barato. Tanto as argilas ( bentonita, zeolita e outras )
como o carvão ativado devem ser tomados entre as refeições
para evitar a adsorção de minerais como Ca, Mg, Zn, Se, etc...de
cargas positivas (cátions).
  A amonia (NH4+) tem uma carga positiva e portanto pode ficar
adsorvida em qualquer argila pois as mesmas tem uma infinidade de
cargas negativas . O que difere uma rgila da outra nesse aspecto é
a chamada C.T.C. ou Capacidade de Troca de Cátions.

Obrigado pelas dicas ( a paciente tem uma cirrose expressiva e as
transaminases estão já muito baixas, o que sugere uma falência hepática
em
andamento)

Estou estudando alternativas que encontrei nos sites tais como Benzoato
de
Sódio (liga-se à Glicina e dá o Hipurato que se liga à amônia)
Encontrei
menções e sites (ver no Google zeolites) que falam do uso de bentonita e
outros minerais. Vou passar este sábado vendo os sites que você
recomentou
de MCM

Não sei se seria novo mas para pacientes com encefalopatia
eu costumo recomendar enemas de café, bastante lactobacilus
acidophilus e outros probioticos. A intenção e fazer os microrganismos
consumirem tanto a amonia quanto outras toxinas tornando-as
inofensivas.
Quero crer que o paciente em questão tem cirrose.
Nesse caso o SAMe é bastante indicado.

A sua linha de raciocínio está correta.
Embora o Zhang não utilize a Curcuma em nenhuma
das suas formulas ( como tambem não utiliza um número
de outras ervas de comprovada eficacia no tratamento de diversos
problemas hepáticos - uma vez eu perguntei o porque e ele
me disse que algumas delas emboram sejam boas e muito usadas
na TCM seria de certa forma redundante na sua estratégia de
tratamento), a Curcuma é na verdade uma grande ferramenta.
É um anti-inflamatório por excelencia ( hepa TITE logo inflamação),
tem reconhecidas propriedades anti cancerígenas e todos sabemos
ao rísco que estamos expostos em função da infecçaõ viral crônica.
Ou seja, encontramos a curcuma em várias formulas hepáticas para
tratamento de hepatites inclusive nas famosas fórmulas tibetanas.
É um pó bem cor de laranja devido ao teor de Curcuma.
A propósito eu tomo curcuma sob a forma de extrato seco com
alto teor de curcumina. Isso é facil de conseguir ou mesmo a curcuma
em pó já quebraria o galho.
É um poderoso anti-oxidante.
Para aumentar a micro circulação hepática o Zhang utiliza os seguintes
extratos na sua fórmula circulation No. 1 :
Safflower flower
peach kernel
dong-quai root
cnidium root
raw rhemania root
Paeonia lactiflora Pall Root
achyranthes root
Chinese thorouhwax root
Chinese licorice ( nesse caso apenas para contrabalançar o efeito de
outras devido ao seu efeito "doce" = antitóxico na TCM)
Ballon-flower root
Ele tem um péssimo modo de listar os ingredientes. Tambem já falei isso
com ele. Ele faz uma miscelanea. Mistura nome cientifico com nome
comum em ingles , com nome comum chines com nomenclatura
latina. Não sei até que ponto isso seria uma forma de impedir a
duplicação
( para 99,99% das pessoas)
Com relação a trifluoperazina seria o caso da gente estudar mais um
pouco o
seu mecanismo de ação e procurar entender um pouco melhor a coisa toda.
Vou certamente dar uma olhada e te informo mais tarde.

No site do Zhang ele ressalta, com muita propriedade, alguns aspectos
descuidados pela medicina ocidental.  Destes, o que me parece mais
importante é a abordagem reducionista, que dá tanta importância ao vírus
que
se não leva em conta o papel do organismo na enfermidade.  Nas palavras
dele

calmodulin inhibitor, upon the microcirculation of cold-stored kidneys.
The
present study points to similar beneficial effects of TFP on the
microcirculation of cold-stored livers; 25 canine livers were preserved
for
24 hr with Euro-Collins' solution (EC) (n = 8), University of Wisconsin
solution (UW) (n = 7), or UW + TFP (n = 10). The stored livers underwent
heterotopic transplantation (HLTX); hepatic-artery and portal-vein
pressure
and flow were monitored; oxygen consumption and extraction were measured
before HLTX and at 15-min intervals after reperfusion, for 1 hr. Mean
hepatic-artery and portal-vein flow (HAF & PVF) prior to donor
hepatectomy
were 172 and 530 cc/min, respectively. Poor HAF and PVF occurred in
EC-HLTX
(mean 35, 175 cc/min, respectively). The damaged EC-flushed livers could
not
compensate to the decreased hepatic blood flow by increased oxygen
extraction (oxygen consumption and extraction, 8.7 vol.% and 48%,
respectively). Light and electron microscopy showed severe liver
necrosis
and periportal hemorrhages. Improved hepatic-artery and portal-vein
flows
were seen in UW HLTX (105 and 254 cc/min), and oxygen consumption and
extraction were 16.4 vol.% and 66%, respectively. Liver biopsy taken
just
before reperfusion revealed well-preserved liver architecture. Liver
biopsy
obtained 1 hr after reperfusion revealed marked edema of the portal
triad,
sinusoid congestion, and hemorrhage. Electron-microscopy biopsies
obtained
during reperfusion at 15-min intervals revealed severe vasospasm of the
terminal hepatic arterioles and progressive damage to the liver
microcirculation. The addition of TFP to the UW-flush solution resulted
in
excellent protection of the liver microcirculation. Marked increase in
hepatic-artery and portal-vein blood flow was noted after reperfusion
(mean
167 and 421 cc/min, respectively (P 0.02 vs. UW: P 0.001 vs. EC). The
recovery of metabolic activity was evident by the high oxygen
consumption
and extraction (25.8 vol.% and 80%, respectively). And serial liver
biopsies
obtained after reperfusion have shown excellent protection of liver
architecture and the absence of hepatic arteriolar vasospasm. Taken
together, these data suggest that the addition of TFP to the UW solution
protects the liver microcirculation by rendering the hepatic
microcirculation insensitive to vasospastic stimuli during reperfusion,
thus
permitting better metabolic recovery after transplantation.

The John Hunter Hospital in Australia has been running a sponsored trial on
the use of a chinese herbal formulation as a treatment for HCV.

A number of people have enquired about this and the following is a letter

Brief Report on Chinese Herbal Medicine Trial for Hepatitis C

The John Hunter hospital has now completed its trial of herbal medicine
therapy for Hepatitis C. This was a placebo controlled trial of a tablet
form of herbal extract for patients presenting with hepatitis C. There were
16 herbs in the tablet preparation and the proportions of the herbs used
will not be made available until the results have been published in the
scientific literature. Suffice it to say here, some of the herbs include
Salvia, Paeonia and Ginseng root.

We took into the trial, patients who had not been treated with Interferon or
patients who had relapsed following treament with Interferon. There were no
other strict entry criteria apart from them having a definitive hepatitis C
antibody and abnormal liver function test on at least 3 occasions.

Patients were treated with 5 tablets 3 times daily with either the active
tablet or the placebo and a small number of patients who, at the end of six
months of treatment on placebo, requested it, and they too received active
tablets for a further 6 months. Our analyisis, therfore, includes all
patients treated blind plus a small number who were treated, knowing that
they were receiving the active tablet after their initial placebo treatment.

None of the patients had any severe adverse side-effects althought two
patients withdrew from the trial because of side-effect symtoms, one of whom
complained of palpitations which stopped on ceasing the tablets and one of
whom had significant bowel symptomatology in the form of diarrhoea and
bloating.

Overall, we found that compliance with the tablets was good.
Our results to date show show there were no effects of the medication on
haematological parameters and that there was a fall in ALT in patients on
active treatment. Six patients normalized their ALT during the treatment
period but we have no evidence that any patient has been cured by the herbs.

At this stage of our analysis, it would appear that the medication has
certainly done no harm and may well be offering some opportunity of
minimising inflammation in the liver of patients with hepatitis C. We need
to do more detailed analyisis or the results at this stage and I could
certainly not recommend the present combination of herbs as the utltimate
management for hepatitis C.

I look forward to publishing our completed results, once we have a full
statistical analysis.

Chinese Herbal Formula
NOTE: Most of these herbals have some known anti-viral properties, but these
are huge doses. Please research this and check with your Dr. and
practitioner before trying this, as they may be harmful to some. Herbal
doesn't mean non-toxic. Also, I believe 'cure' means the same thing as
'responder' with IFN, ie: there are normal lft's or pcr, but no decrease in
disease progression. I have not seen one case or heard of one case of 'cure'
but suspected long term remission has been achieved by some methods.

In a report in the Chinese Journal of Integrated Traditional and Western
medicine (1994), a claimed rate of cure of 56%, with most other patients
showing improvements, was obtained when the following formula was
administered to treat hepatitis C:

astragalus: 30 grams
salvia: 30 grams
forsythia: 30 grams
red peony: 30 grams
ho-shou-wu: 15 grams
crataegus: 15 grams
moutan: 15 grams
gardenia: 15 grams
dandelion: 15 grams
bupleurum: 10 grams

The herbs are decocted and the amount indicated here is taken in two divided
doses each day, for three months. The formula can be modified to address
specific symptoms by adding additional herbs (e.g. for pain in the liver
area, loss of appetite, or abdominal distention). As with treatments for
hepatitis B, the formula contains herbs for treating damp-heat (forsythia,
gardenia, dandelion, and bupleurum), blood stagnation (salvia, red peony,
crataegus, moutan), and deficiency of qi and blood (astragalus and
ho-shou-wu).

Due to the long course of therapy, one may wish to substitute dried
extracts: a dose of three teaspoons (9grams), three times daily of this
formulation should produce similar response [about 27 grams per day of dried
extracts is roughly equivalent to a decoction of 160 grams of crude herbs,
somewhat less than is recommended in the above clinical trial; the patient
cost of this treatment is about $500 per three month course]. some patients
may experience loose stool or diarrhea in response to this therapy (e.g. ho
shou wu, gardenia, and dandelion can act as laxatives), thus one may need to
adjust the formulation somewhat if this reaction occurs andpersists. It is
not known if good results can be obtained by substituting powdered herbs for
the extracts. If one wished to substitute tablets (which are comprised
mainly of powdered crude herbs), Salvia Shgou Wu contains the salvia
(extract), astragalus, crataegus, ho-shou-wu and red peony, while Nuphar 14
contains the moutan, bupleurum, and gardenia, as well as additional salvia,
and Red Peony Tablets contains the dandelion and forsythia, as well as some
additional red peony (extract). The use of these tablets will result in less
relative dosage of the herbs than in the decoction formula, but there are
additional herbs that would be expected to support their actions. One might
consume three tablets of each formula each time, three times daily [total:
27 tablets per day; patient cost is about $300 per three month course].
Using the tablets, one would expect to need a somewhat longer therapy than
the higher dose decoctions or granules.
The dose of one or more of the formulas could be increased (e.g. to four
tablets each time), if desired.

Until clinical work with hepC and Chinese herbs is carried out in the United
States with available materials that are in a form suitable for
administration, it may be difficult to convince medical practitioners and
patients to try this method. Because the herbs are non-toxic, some patients
may wish to utilize this therapy in place of, or in addition to, treatment
by interferon. It is reasonable to begin collecting information from such
patients to provide case histories in an effort to eventually develop a
clinical trial.

OTHER CHINESE HERB THERAPIES FOR HEPATITIS C

Chinese medical treatment is often based on the model of treating hepatitis
B.
Even though the viruses are quite different, the fact that the diseases are
caused by viruses that ultimately damage the liver remains the same, and the
herb therapies are thus similar in nature---at least until further
diagnostic information is available. Hepatitis B is treated by herbs for the
traditional categories of pathological disturbance labeled as damp-heat, qi
and blood stasis, and for qi, blood, and yin deficiency.
Herbs are also given according to specific manifestation of the disease and
underlying constitutional factors. Chinese tonic herbs have been shown to
enhance interferon production and thus may substitute for interferon therapy
(six months of alpha interferon injections is the standard procedure). Use
of exogenous alpha interferon is often unsuccessful and, sometimes, there is
a rebound of hepatitis C viremia after apparently successful treatment with
this drug.

Herb therapy provides its effects through more mechanisms of antiviral
action than simply increasing interferon, and it can be pursued for a longer
time, due to absence of substantial side-effects. Cures for hepatitis B have
been repeatedly reported in the Chinese literature and are accomplished by
administering non-toxic herbs and vitamins.
ITM is currently involved in evaluating a formulation for treatment of
hepatitis B in China.
A related protocol, being used in the U.S., is to consume 9 tablets of the
Seven Forests Eclipta Tablets (or 4 of the larger tablets of White Tiger
Baicalcumin) and 1 tablet of the White Tiger Quercenol each time, three
times daily for 12 weeks; favorable responses for hepatitis B have been
informally reported to ITM from both the American cases and the initial
Chinese research (with 40 patients).

Other studies reported in the Chinese medical literature include these:

The details of the treatment were not specified. Patients were given
different formulas according to presentation of constitution and symptoms;
typical herbs used included smilax, scute, dictamnus, salvia, epimedium,
loranthus, and lycium fruit. Inosine and vitamins were also given orally. It
was calimed that 20 of the 33 patients (60.6%) so-treated were cured.

198-201).
The patients selected for treatment were suffering from aplastic anemia and
had probably become infected by hepatitis C as the result of blood
transfusions. The patients were treated according to differential diagnosis,
with high-dosage herb combinations. As an example, for patients with
symptoms such as pallor, lassitude, anorexia, nausea, abdominal fullness,
and thin stools, the prescription included:
25 grams pseudostellaria,
25 grams astragalus,
10 grams citrus, 10 grams tang-kuei,
12 grams cardamon,
20 grams peony,
20 grams bupleurum,
25 grams polygonatum,
20 grams coix,
20 grams plantago seed.

Patients also received intravenous vitamins and other nutrient factors.
Among 21 patients with hepatitis C, 17 were reportedly improved by the
treatment, but only 3 were said to be cured.

Patients who had a history of blood transfusion and who tested positive for
hepatitis C were divided into two groups; the control group received
alpha-interferon and the herb group received herbal decoctions (depending on
presenting symptoms and signs). As an example, for those classified as
presenting liver qi stagnation and spleen deficiency, the formula was:
15 grams bupleurum,
12 grams hoelen,
10 grams atractylodes,
10 grams codonopsis,
10 grams peony,
6 grams chih-ko,
6 grams gardenia,
6 grams curcuma,
5 grams licorice.

For those classified as having accumulated heat toxin, the formula was:
15 grams lithospermum,
15 grams hu-chang,
15 grams forsythia,
12 grams scrophularia,
12 grams gardenia,
10 grams raw rehmannia,
10 grams moutan,
10 grams red epony,
6 grams curcuma,
5 grams licorice.
Other herbs might be added to these base formulas for treating specific
symptoms.
Among the 32 persons treated by herbs, 4 were cured and 25 others were
improved. With the alpha interferon, 2 of 32 paitents were cured, and 19
others were improved.

CHINESE HERBAL MEDICINE IMPLICATED IN LIVER FAILURE

Another case of acute hepatitis and liver failure linked to consumption of
the Chinese herbal medication, ma-huang, has been reported by clinicians at
the University of California at San Francisco (UCSF).

A 63-year-old Chinese woman developed fulminant liver failure requiring
transplantation following the use of the mixture of 12 plant species. She
was one of three people who presented to UCSF with acute hepatitis
associated with Chinese herbal medications during a two-month period.

"The patient transplanted did not have a history of chronic liver disease
nor alcohol consumption," Eric M. Yoshida and colleagues from UCSF wrote
("Chinese Herbal Medicine, Fulminant Hepatitis, and Liver Transplantation,"
American Journal of Gastroenterology, December 1996;91(12):1647-1648).
"Viral studies, however, revealed seropositivity for hepatitis B surface
antigen and hepatitis B core antibody (HBcAb) IgG; serum hepatitis B virus
DNA was 511."

Lake et al. noted that serology for HBcAb IgM was negative. Serologies for
hepatitis C, hepatitis A IgM, and hepatitis D, likewise, were negative.

When first admitted to the hospital, the woman was notably jaundiced and
encephalopathic with no stigmata of chronic liver disease, and initial
clinical impression was of fulminant viral hepatitis. A history was taken
and the woman acknowledged taking the Chinese herbal medication ma-huang for
a one-week period four weeks before admission.

Coagulopathy, jaundice, and encephalopathy progressively worsened requiring
endotracheal intubation. Liver transplantation was performed and gross
inspection of the explanted liver revealed marked collapse and very small
regenerative nodules. Histologic examination of the explant revealed massive
centrilobular necrosis with periportal bile ductular proliferation and
inflammation.

The UCSF cases are not the first to implicate ma-huang in the development of
acute hepatitis and liver failure. In 1996, Nadir et al. also reported such
a case (Am J Gastroenterol 1996;91:1436-1438).

"Our transplanted patient would have had a fatal outcome, which is
infrequently reported, had an allograft not been available," Yoshida et al.
wrote.

"The recent experience at UCSF demonstrates that liver injury from
traditional Chinese medicines does not always resolve, and urgent
transplantation may be required. It remains to be seen whether or not the
recent cluster of cases at UCSF represents a trend that grows with the
increasing use of 'alternative' medicines."

Chinese Herbal Medicine as Effective as Conventional Therapy for Treating
Cirrhosis in Rats
(Apr '96)

WESTPORT, Apr 08 (Reuters) - The authors of a study that evaluated the
effectiveness of four therapies, one of which was traditional Chinese herbal
medicine, in a rat model of cirrhosis have found that all four therapies
were effective. Drs. Zhang Manna and Song Guopei of Jilin, China, and Dr.
Gerald Y. Minuk, of the University of Manitoba, describe their work in the
April issue of Gastroenterology.

First, they induced liver fibrosis in Wistar rats by administering carbon
tetrachloride. Then the rats were randomized to one of four treatment
groups: ciprofloxacin, selenium plus vitamin E, hepatic stimulator
substance, and a traditional Chinese herbal medicine. The latter consisted
of boiled turtle shell, pangolin scales, leeches, ginseng, thorowax and
other "minor" ingredients.

Drs. Manna, Guopei and Minuk say all four therapies decreased the amount of
hepatic fibrosis, but the most effective therapy was the Chinese herbal
concoction.

Treating Alcoholism
The only treatment for alcoholic cirrhosis is to stop drinking. Individuals
with alcoholic cirrhosis are typically malnourished and require increased
calories and rigorous nutritional support, which can improve survival rates.
Drugs under investigation for treating alcoholic cirrhosis include the
following:
Propylthiouracil and colchicine, which inhibit deposits of collagen, the
critical protein building block in connective and scar tissue.


Drugs that block factors in the immune system, called thromboxanes, may play
an important role in the inflammatory process that kills liver cells in
alcoholic cirrhoses.


A 2000 study on polyenylphosphatidylcholine (PPC), an antioxidant extract
from soybeans, showed that it could halt, or perhaps even reverse,
progression of liver fibrosis (scarring) in alcoholics. More research is

Treating Chronic Hepatitis B or C
Drug treatments for chronic hepatitis B and C are aimed at reducing or
preventing liver damage and boosting or modifying the immune system to
promote its attack on the viruses. [For more information, see
Well-Connected, Report # 59, Hepatitis.]

Investigative Drugs for Hepatitis C.
Amantadine. Amantadine (Symmetrel) is a drug commonly used for Parkinson's
disease, but which may have some antiviral effects. One 2000 study reported
that in combination with interferon, it improves the quality of life more
effectively than interferon alone. In another study, it was helpful in
combination with interferon and ribavirin for hepatitis C patients who had
failed interferon.


Ursodeoxycholic Acid. Ursodiol, or ursodeoxycholic acid (UDCA), a drug
ordinarily used for primary biliary cirrhosis, improves aminotransferase
levels. It has no effect against the virus, but may be useful in combination
with interferon.


Protease Inhibitors. Protease inhibitors (similar to those used for HIV) are
under investigation for hepatitis C patients who fail other treatments.


Interleukin. Researchers are also looking at interleukins 10 and 12, immune
factors that may enhance the immune response to hepatitis C.

Antioxidants. Antioxidants may help reduce liver injury from damaging
particles known as oxygen free radicals. Among the antioxidants being
investigated are an agent known as a histamine type-2 receptor agonist
(Maxamime), N-acetylcysteine (an amino acid byproduct), and vitamin E. One
very small study reported benefits from a combination of the antioxidants
alpha lipoic acid (thioctic acid), silymarin (found in milk thistle), and
selenium. In addition to protecting against free radical damage, these
agents may have antiviral and immune boosting qualities.


Glycyrrhizin. Glycyrrhizin is a compound in licorice. Studies are reporting
that it reduces liver enzymes (although has no effect on viral count).
Primary Biliary Cirrhosis
Ursodeoxycholic Acid (UDCA) and Drugs Used to Slow Progression. At this time
no medication can cure primary biliary cirrhosis. Ursodiol, ursodeoxycholic
acid (Actigall), or UDCA has been the standard drug used for primary biliary
cirrhosis. A number of studies have reported that it slows progression and
improves survival in patients. Of concern are long-term studies conducted in
the last three years reporting no survival advantages. One of these reports,
which was a 1999 European analysis of 11 studies, also found no effect from
the drug on scarring, bleeding, or protein levels. Nevertheless, UDCA is
currently the only agent recommended for treating primary biliary cirrhosis.
It has no effect on symptoms, including itching and fatigue. Some drugs,
such as colchicine, corticosteroids, or immunosuppressants, are being
investigated for use in combination with UDCA. To date, combinations with
methotrexate or budesonide, a corticosteroid, have shown some additional
benefits. Long-term controlled trials are needed to determine the value of
UDCA alone or with other agents.

Agents for Itching . Itching is a major problem with this disease and a
number of agents have been used or investigated:
Itching can be relieved by taking cholestyramine with meals.


Low doses of the drug naltrexone relieved itching in one study; high doses
of this drug are toxic to the liver.


Phototherapy, which uses light, may also reduce itching.


Methotrexate, an anti-inflammatory drug that suppresses elements of the
immune system, has been shown to reduce itching. It was hoped that the drug
might improve liver disease. A small 2000 study reported, however, that it
improved liver tissue health in only 18% of patients and, in fact, the
disease accelerated in 50% of the patients.


In one study an antioxidant compound (Bio-Antax) plus coenzyme Q10, another
antioxidant, relieved itching and fatigue.
Agents for Impaired Fat Absorption . Because primary biliary cirrhosis
affects fat absorption, patients may need high doses or injections of
important fat-soluble vitamins, including K, D, A, and E. For steatorrhea
(in which the feces contain excessive fat) agents called medium-chain
triglycerides may be helpful.
Treatments for Other Causes of Cirrhosis
Secondary Biliary Cirrhosis . Secondary biliary cirrhosis caused by blockage
in the bile ducts can be relieved by surgery.

Autoimmune Hepatitis. Autoimmune hepatitis is treated with corticosteroids
as standard agents and also possibly immunosuppressants, such as
azathioprine and cyclosporine A.

Hemochromatosis. For hemochromatosis, weekly bleedings (phlebotomies) may be
performed until iron levels are normal, then repeated as needed. If
treatment is given before cirrhosis develops, life expectancy may be normal.

Wilson's Disease . D-penicillamine is the drug most used for Wilson's
disease.
